Start with a piece of 6" x 7" C/S
On the 7" side score at 3/4", 4-1/4", 5"
on the 6" side score at 2", 2-3/4", 4-3/4", 5-1/2".
Use the picture diagram to cut on the solid lines, and score on the dotted lines.
Use tear tape on the 3/4" side, and fold box.
Round the corners on the flap.
